The following ICT security incidents should be reported to the ICT Division immediately:

a) Information is lost, disclosed to unauthorized parties or, suspected of being lost or disclosed to unauthorized parties;

b) Information systems are used without authorization or suspected of being so;

c) Passwords or access control mechanisms are lost, stolen or disclosed, or suspected of being lost, stolen or disclosed;

d) Unusual system events occur such as lost files, frequent system failures and misdirected communications; and

e) Intrusion attempts, fraud and unexpected incidents occur.
